support staff member or an entire school. Each honoree will receive a personalized
certificate recognizing the contribution made in his or her name, PLUS a "you are
amazing!" magnetic dry eraser as a daily reminder of how much you appreciate them.
Certificates will be delivered the week of June 4th.
Proceeds from Thank A Staff will benefit Northville Educational Foundation's
Innovative Teaching Grant program and other professional development programs.

Please fill out one line for each of the certificates you order.
If you are giving one teacher more than one certificate, still complete a line with their name for each certificate you order for them.
For the "In Appreciation of" column, please use the full first name AND last name of the staff member, and not just their title (i.e. Mr., Mrs.). This will help ensure the certificates get to the person for whom they are meant.
